- Athletic Emergency Card (required for all spring athletes)
In addition to the medical record on file in the Nurse’s office, we require an Athletic Emergency Card. After a review by our athletic trainer, the form is given to the student-athlete's coach and it travels with them to all practices and contests.
This form requires all sections to be completed in their entirety. Please mark all fields and then print the form adding parent/guardian signature in the two places requested.

- Interval Health Form (required for all spring athletes)
In order to keep your child’s health record up to date to provide the best possible health care while participating in interscholastic athletic activity, a health history update is required unless the school medical examination is conducted within 30 days of the start of a season.

- Code of Conduct/Training Rules/Assumption of Risk Form (not required if you were a Fall or Winter athlete)
Horace Mann athletes are held to a high standard of conduct, training rules, regulations and responsibilities that are unique to an athletic program. Student-athletes and parent/guardians are encourage to read this document together as it requires joint acceptance and each of your signatures.

- Concussion Policy and Acknowledgment Form (not required if you were a Fall or Winter athlete)
The Centers for Disease Control and Prevention estimates that 300,000 concussions are sustained during sports related activities nationwide, and more than 62,000 concussions are sustained each year in high school contact sports. Second-impact syndrome occurs when a person sustains a second concussion while still experiencing symptoms of a previous concussion. It can lead to severe impairment and even death of the victim.
It is imperative that athletes, coaches, and parent/guardians are educated about the nature and treatment of sports related concussions and other head injuries.

- Heat Index Procedures and Weather Safety Policy
In recent years, studies have cited a marked increase in player injury and death being brought about by heat related causes. Heat stroke has become a major concern. To address these concerns here at HM and to minimize the threat or need to cancel practices on short notice, we schedule only early morning practices. Some teams have opted to include weight training or classroom instruction as an addition to their schedule. These added activities will take place in air-conditioned environments. We will continue to use the NYSPHSAA standard for Heat Index Procedures when determining practice conditions.
